Our service is for children and young people up to the age of 18 who have life limiting conditions and complex healthcare needs, and their families and children who attend Old Park and Pens Meadow Special School
The team provides support usually in the child’s own home or at school. The team includes specialist children’s nurses and health care support workers and is supported by a Citizens Advice Bureau worker.
The team offers support to families and carers to assist in the children’s care including training and advice.
Referrals can be made by a variety of professionals.
