The keyworker service is a multi-disciplinary team of professionals from varying backgrounds including; nursing, education, psychology, early years, social care and occupational therapy. They support any young people (0-25) with a Learning Disability and/or Autism who are currently an in a mental health inpatient facility or on the Dynamic Support Register (therefore at risk of admission to a mental health inpatient facility, risk of placement breakdown and/or risks of criminality).
The keyworkers utilise their knowledge and experience to empower young people and families.
The keyworker service aims to navigate, empower, support in access and stabilise.
We aim to:
- help coordinate services and bridge gaps
- support young people and their families/carers in navigating complex systems
- empower young people and families
We work holistically with young people and families, putting their goals and wishes at the centre of our work
We currently accept referrals via the Dynamic Support Registers.
We have recently commenced a 12 month pilot of an early intervention model called Green Team where we will be looking at how we can prevent escalation onto the dynamic support register and support young people with learning disabilities and autism to live well in the community.
